Medical Visa 

Requirements: 

1. (i) Hard copy of application form after uploading on the Indian visa website.
2. (ii) Two recent passport size photographs.
3. (iii) Passport(New and old) with photocopy including all pages with entries/visa granted with photocopy.
4. (iv) Copy of Shenasnameh.
5. (v) A referral letter or preliminary medical advice from Iranian hospital, recommending specialized medical treatment abroad.
6. (vi) Letter from the medical hospital in India, indicating appointment or showing willingness to undertake treatment also mentioning approximate expenditure for the treatment.
7. (vii) Financial statement showing that sufficient funds are available. 

Note: 

Attendant of an applicant who is granted `Medical’ visa will be granted `Medical Attendant’ visa provided the family members are otherwise eligible for grant of such a visa.  The validity of such visa shall be co-terminus with the validity of the visa of the principal visa holder or for such shorter  period as may be considered necessary by the Indian Mission.
